The coaching quiz collects your name, email, phone number if provided, and the goals and preferences you choose to share. Jasmine uses these details to respond to your inquiry and prepare for your discovery call. Sending an inquiry does not subscribe you to marketing.

What to share

Please don’t include medical records, diagnoses, payment details, or other sensitive information in the quiz. For a teen athlete, a parent or guardian should provide their own contact details.

Form delivery and booking

FormSubmit processes the inquiry and forwards it to jasmine@valkyrierisingtraining.com. Its spam checks may ask you to complete a CAPTCHA. Google Calendar handles appointment selection; its own privacy terms apply when you open the booking page. Your appointment is confirmed only after you complete the booking.

Your draft

The quiz saves a draft in this browser tab’s session storage so you can go back and edit. It is not sent to Jasmine until you submit. Close the tab or use “Clear my draft” to remove it. Session storage must be available for the save feature to work.

Questions or deletion requests

Email jasmine@valkyrierisingtraining.com to ask about information you have shared or request its removal from Valkyrie Rising’s records. Hosting, email, spam-protection, and booking providers may retain their own logs under their policies.
